Parent/Guardian Agreement
As the parent/guardian of the registered, minor player, you agree to abide by all the rules of Neusport Football Club, the leagues in which Neusport teams compete, US Youth Soccer and its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with soccer programs and activities, you release, discharge and/or otherwise indemnify Neusport Football Club, US Youth Soccer and its affiliated sponsors and organizations, their employees and associated personnel including the owners of fields and facilities utilized for the programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the program and/or being transported to or from the same, which transportation you authorize.
Consent for Medical Treatment
As the parent/guardian of the registered, minor player, you give your consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well-being of the player.
Financial Agreement
As parent or guardian, you are making a commitment to pay all fees and dues. You will be expected to pay the fees when due in order to maintain your child’s player status as a “player in good financial standing.” Those who fail to pay or maintain account will have their player’s privileges to participate in practices, clinics, games, tournaments, and team activities suspended until their financial responsibilities are met. This policy will be enforced after any payment is over 30 days past due and could result in your child’s player’s card being pulled and suspended until your account is brought current.
If you need additional time or you have difficulty paying the fees, it is your responsibility to contact Neusport Football Club. Fundraising and sponsorships may be available to help defray financial obligations. Scholarships are available on a limited basis and if a player qualifies, can be used to cover training fees only. Other fees incurred remain the obligation of the individual player. Any information related to personal finances will be kept in strict confidence.
Financial Terms and Conditions
· The $190, one-time annual registration fee is non-refundable.
· Acceptance to Neusport Football Club is not guaranteed until full payment of the annual registration is received.
- Training fees (U6-U8: $93; U9-U10: $293; U11: $618; U12-U18: $863) must be paid on the dates agreed to by the Parent/Guardian of the player and Neusport Football Club.
- If parent/guardian is unable to pay training fees on the dates agreed to by both parties, it is the parent/guardian’s responsibility to submit written notification to the Neusport Club Administrator at customerservice@neusport.org. It is also the responsibility of the parent/guardian to make arrangements to pay the outstanding fees within 15 days of the original due date.
- If you should incur a past due balance on your account and do not contact the Neusport Club Administrator in writing, your account will be automatically charged $20 for every 15 days that your payment is late.
- After 30 days, your son’s or daughter’s player’s card will be pulled, excluding them from game and tournament competition.
- After 60 days, your son or daughter will not be able to practice and will not be allowed to compete in games or tournaments with the team.
- After 90 days, your account will be handed over to collections.
- If a player chooses to leave Neusport for any reason at any time after signing, there will be NO refunds. Upon choosing to leave Neusport, your account must be paid up-to-date before a release will be signed.
- Partial refunds based on player season ending injuries and moves outside of Clark County will be considered on a case-by-case basis. If a player is seriously injured or disabled and is unable to play or practice for more than two months, they are eligible for a refund for the time they are unable to play on a pro-rated basis. In order to qualify for a refund, a doctor’s letter must be provided to the Club Administrator stating the type of injury and length of time the player is expected to be out. Any awarded refunds to club fees are subject to a $25 penalty and club fees will be prorated.
- Uniforms, league fees, camps & clinics (which may be mandatory for some teams), tournament fees, and travel costs are not included in the registration or training fees.
